Overview
Lead Capsule offers a pre-configured Granuflo lead vertical template, which allows you
to be up and running with your Granuflo lead vertical in a few mouse clicks. The Granuflo template creates all the necessary fields for up a basic Granuflo lead. The fields created, when setting up a Granuflo vertical using
the vertical template, will create the following Granuflo lead fields
automatically.

- Email Address
- Address 1
- Address 2
- City
- State
- Zip Code
- Phone
- Date Of Birth
- IP Address
- Has Attorney?
- Notes
- Were you hospitalized?
- How long were you hospitalized?
- Verified?
- Are you on Dialysis?
- Have you taken Granuflo or Naturalyte?
- Have you suffered any of the following injuries?
- What year?
- How soon after your dialysis did you suffer this injury?
- Prior diagnosis of a heart attack or stroke?
- Any pre-existing injuries that may have caused your injury/dx?
Please note: Even though these Granuflo lead fields are
created, they do not have to be used. You may also add in your own custom Granuflo lead
fields.
